Supreme Court applications typically require a Notice of Application (Form F31) together with the affidavits and other documents relied upon. Form F30 is the standard affidavit form.

- instructions for filing and serving the materials
Not included: court appearance, service, filing fees, responding to further materials, negotiation with the opposing party, supplementary affidavits or ongoing representation.
